Carolina Caring is seeking a Nurse Practitioner to deliver care to patients in the Home Base Primary Care program.
Must be able to travel to patient’s homes to provide care in their residence.

Minimum Experience:
A minimum of two years in a Primary Care setting. Gerontology experience or certification preferred. Experience with CMS Chronic Care Management Services preferred.
Required Degree:
Graduate of an accredited school of nursing with a Master’s degree in nursing and appropriate advanced training required for licensure as a registered nurse practitioner. Current, unrestricted North Carolina Nurse Practitioner license. Must maintain CPR certification. Must have active DEA certificate.
