About Our Advanced Cardiac Care Program

As the first program of its kind in North Carolina and one of the first in the nation, we are proud to have received the Palliative/Hospice Heart Failure certification from the American Heart Association®. Our program revolutionizes the management of advanced heart disease through innovative at-home care, aiming to give you the tools and resources needed to fill each day with purpose. We are here to help care for your heart.

Developed in collaboration with the National Partnership for Healthcare and Hospice Innovation and the American Heart Association, our Advanced Cardiac Care Program addresses the critical need for palliative services in advanced heart failure management. As part of this program, we work closely with your doctors to create a personalized care plan that manages symptoms, minimizes emergency room visits and, most importantly, improves your quality of life. Specialized nursing care is provided wherever you’re most comfortable—whether that be a hospital, assisted living facility or your home.

Advanced Cardiac Care Program Coordinator

Rachel Hill, with over 18 years of nursing experience, specializes in cardiac and critical care. She has been leading our Advanced Cardiac Care Program since September 2023. Rachel holds degrees from Catawba Valley Community College, the University of North Carolina at Charlotte, and Lenoir-Rhyne University. Her extensive experience includes roles in cardiac and critical care nursing and as a nursing instructor. Rachel is certified in critical care nursing and is a member of the International Honor Society of Nurses.
